This online MDS Admission Form is applicable only to candidates allotted to Indira Gandhi Institute of Dental Sciences (IGIDS), Kothamangalam, through the applicable CEE/KEAM allotment process.

Candidates are requested to read the instructions carefully and keep the following information and documents ready before starting the application.

Documents & Information to Keep Ready

Please keep clear and valid copies/details of the following available before filling out the form:

1. Recent passport-size photograph of the candidate.
2. Aadhaar Card / Aadhaar Number.

NBE NEET-MDS details, including:

1. Application Number
2. Roll Number
3. Score
4. Rank
5. Score Card details

CEE/KEAM allotment details, including:

1. Application/Registration details
2. Allotment Memo
3. Allotment details
4. KEAM/CEE Acknowledgement Page and Fee Receipt, if required for entering application/allotment details.

BDS academic details, including:

1. Degree Certificate details
2. Internship Certificate details
3. S.S.L.C. / Plus Two details, wherever required
4. Dental Council Registration details, including current registration/renewal information.
5. Migration Certificate details, if applicable.
6. KUHS Eligibility Certificate, if applicable.

Category/Community details and certificates, if applicable:

1. SC/ST/OEC
2. Non-Creamy Layer Certificate, wherever applicable
3. Income Certificate, wherever applicable
4. NRI-related documents/details, if applicable.
5. BLS Certificate details, if required.
6. Physical Fitness Certificate, if required.
7. Vaccination Certificate details, if required.
8. Bank account details, including the candidate’s savings bank account details, Aadhaar linkage and PAN linkage, where required.
9. Extra-Curricular Activities Certificate details, if applicable.
